Initial Wellness Consultation

Service Description

Your initial wellness consultation is designed to understand your health goals, symptoms, medical history, lifestyle, and areas of concern. During this visit, we will discuss your priorities, review appropriate wellness options, and determine whether Bloom & Sage Wellness is a good fit for your needs. Recommendations may include lab evaluation, weight management support, hormone health guidance, metabolic wellness planning, peptide therapy considerations, or ongoing care options when clinically appropriate. To provide a more thoughtful and personalized consultation, submitted intake forms are reviewed prior to your appointment. When clinically appropriate, a lab order may be sent in advance so testing can be completed before your visit. This allows your provider to review available results during the consultation and create more meaningful, individualized recommendations. Initial consultation includes provider review of submitted intake forms and lab ordering when clinically appropriate. Lab testing fees, medications, supplements, and ongoing membership services are billed separately unless otherwise stated.
